Title: Family Service Coordinator (Early Intervention)

Amplified Therapy Inc. is a family-owned pediatric healthcare provider specializing in early intervention and school-based therapy services across New Mexico. As a certified NM Family Infant Toddler (FIT) provider, ATI delivers high-quality, no-cost services to families with children from birth to age three who are at risk of developmental delays or disabilities.

 

Position Summary:

The Family Service Coordinator (FSC) plays a vital role in the NM FIT Early Intervention program. This position serves as the single point of contact for families, helping them access and coordinate early intervention services as outlined in the Individualized Family Service Plan (IFSP). The Family Service Coordinator (FSC) ensures timely service delivery, supports families through the evaluation and eligibility process, and facilitates IFSP development and implementation.

 

Family Service Coordinator (FSC) Qualifications:

  • Bachelor’s degree in social work, counseling, psychology, nursing, special education, early childhood education, or a closely related field.
  • OR a bachelor’s degree in another field plus two (2) years of direct experience serving individuals with disabilities and/or families.  

Family Service Coordinator Duties and Responsibilities:

  • Serve as the primary contact for families receiving early intervention services.
  • Coordinate evaluations, assessments, and eligibility determinations.
  • Facilitate and participate in IFSP meetings and multidisciplinary team discussions.
  • Participate in staff development, supervision, and community outreach.
  • Represent ATI in community events, trainings, and meetings.
  • Adapt to various service delivery settings including homes, clinics, and community environments.
  • Travel within the assigned county of service to complete family visits.
