Our skilled nursing facility in Lancaster, PA, is looking for a passionate and detail-oriented Registered Nurse Assessment Coordinator (RNAC)/MDS Coordinator to join our team!
This full time role offers an opportunity to make a meaningful impact by ensuring quality care through comprehensive resident assessments and care planning in alignment with state regulations and our facility's mission.
Shift: 8a-4p
Your Role and Responsibilities:
Conduct routine resident assessments according to schedules or as needed for condition changes, hospital discharges, and other events.
Accurately observe, assess, and communicate condition updates to the appropriate team members.
Collaborate with Social Services, Activities, and Dietary departments to ensure timely completion of assessments and CAAs with proper documentation.
Complete MDS sections, quarterly reviews, CAAs, and care plans as required.
Develop and update resident care plans based on assessment protocols.
Prepare and share the MDS schedule with the interdisciplinary team.
Transmit MDS data to the state weekly (or as needed) while maintaining meticulous records.
What We’re Looking For:
Current Registered Nurse (RN) licensure in Pennsylvania.
Proven experience in long-term care, with a strong background as an MDS assessment coordinator.
