• Contract Duration: 13 weeks\n
• Shifts per Week: 3 (blocked schedule: Friday, Saturday, Sunday; 12-hour day shifts, 36 hours per week)\n
• Estimated Weekly Pay: $1,935 - $2,150\n
• Employment Type: Travel Nursing Contract (On-Site)\n

\n\nRegistered Nurse Qualifications\n

\n
• Minimum 12 months of recent Home Health RN experience (within the last 3 years)\n
• Proficiency with HCHB (Homecare Homebase) – required\n
• OASIS documentation experience – required\n
• Active Wisconsin or Compact Registered Nurse License – required (must be in hand at time of submission)\n
• Current BLS certification – required\n

\n\nHome Health RN Responsibilities\n

\n
• Provide skilled nursing care to patients in their homes across the Greater Milwaukee area\n
• Complete three patient admissions per day and approximately two additional home visits daily\n
• Utilize HCHB and OASIS for accurate, timely clinical documentation\n
• Collaborate with interdisciplinary healthcare teams to develop and implement individualized care plans\n
• Educate patients and families on disease management, medications, and self-care techniques\n
• Ensure compliance with all regulatory, clinical, and safety standards\n
