Job Details

• 36 weekly hours

Job Requirements

• RN license required
• Nursys verification within 30 days prior to start
• Current license and primary source verification within 30 days prior to start
• Completion of FACIS Questionnaire if any disciplinary actions
• Completion of Ascension Color Vision prior to start and updated every 3 years
• Completion of Ascension Core Competencies within a year of start, updated yearly
• NIH Certification updated yearly or every 2 years depending on group
• Completion of Ascension EKG Exam prior to start with passing score
• Completion of Ascension Onboarding Requirements Education Attestation prior to start
• Completion of Ascension Nursing Contract Worker Onboarding prior to start
• Completion of Ascension Nova StatStrip prior to start
• Completion of Ascension Cerner Test-Out prior to start
• Submission of Ascension Badge Photo prior to start
• Documented proof of two MMR vaccines or positive IGG Titer, no declinations accepted
• Fit Test required prior to start, updated annually
• Documentation of Hepatitis B vaccination series completion or declination
• Influenza Vaccine required for flu season, exemptions accepted with mask requirement
• Positive IGRA or PPD results with X-ray within a year and TB Questionnaire updated annually
• Statement of good health or completed physical exam prior to first assignment, updated annually
• Driver's License/State ID must be valid and match address requirements
• 2 Step PPD OR IGRA within required timeframes prior to start
• Documented proof of two Varicella vaccines or immunity, no declinations accepted
• Tdap valid within 10 years, no declinations accepted
• Ascension Profile submission required
• At least two professional references prior to first assignment
• Completion of Ascension Criminal Background Check within 30 days prior to first assignment, updated yearly
• Ascension EPLS run within 90 days of start, rerun if break in service >90 days
• Ascension Skills Checklist clinical only, completed within one year of start, updated annually
• Ascension 12 Panel Drug Screen within 30 days prior to start, updated annually
• Ascension Education Verification within 90 days of start
• Ascension Confirmation of Assignment upon initial assignment and updates
• Third-party employment verification for past seven years, updated yearly
